Novel Use of the Piranha Morcellator To Evacuate Fungal Debri Within the Bladder

Background: This is a 70 year old male with a history of diabetes mellitus, hypertension, coronary artery disease and BPH who has intermittent passage of obstructing white pasty substance from his urethra
Procedure: Office cystoscopy revealed a large, 6 cm whitish mass arising from the dome of the bladder, with CT Enterography being negative for a colovesicular fistula. He was brought to the operating room. Using a 24 Fr Wolf resectoscope sheath and Piranha morcellator, the soft, mobile whitish mass was evacuated. Cystitis features on the posterior wall of the bladder was biopsied
Results: Pathology of the cystitis features revealed cystitis glandularis while the mass that was evacuated from the morcellator revealed multiple yeast and pseudohyphae consistent with Candida species fungal debri
Conclusions: For soft, mobile masses, the Piranha morcellator provides an alternative means of evacuating this from the bladder.
